Bringing Care Closer To Communities builds on the first and only allied health-led Diabetes Telehealth model, which has already saved over a million kilometres of travel for Aboriginal type 2 diabetes patients. Through cultural co-design, Diabetes WA is reducing inequity in access to care by addressing barriers to engagement with technological solutions. Aboriginal people are three times more likely to develop type 2 diabetes, seven times more likely to die from diabetes and 38 times more likely to have lower limb amputations due to diabetes than non-Aboriginal people. With this project, Diabetes WA is leading the way in addressing health inequity in rural and remote Aboriginal communities, and is applying learnings to other telehealth services in Australia.
