SMS-based support to help people with chronic diseases lead healthier lives

Chronic diseases are the leading cause of premature death, responsible for 63% of all deaths worldwide and creating substantial economic and social burdens for individuals and families. Fewer than 1 in 3 patients use presently-available in-person support services. TEXTCARE removes barriers to leading a healthy life for people living with chronic disease through an SMS-based behaviour change program. The system uses computerised algorithms to compose personalised messages and deliver proven educational and motivational content in a way that's convenient for patients and that scales easily. This project aims to help the half of all Australians living with a chronic disease, and could save the Australian healthcare system nearly $5 million.
